GERIATRIC NP/Clinical Supervisors Needed!
Openings in NYC, Westchester County, Staten Island and the 5 boroughs
This NP will act as a consultant on general medical issues for Care Managers and Primary Care Physicians. This will include providing and coordinating medical care to patients in nursing homes in conjunction with PCPs and Medical Directors. You will also make home visits to complex patients and advise Care Managers on developing their care plans.
This position requires NY NP licensure, registration and DEA certificate plus at least one year of NP experience and 3 to 6 months specifically in geriatric care. ISNP experience preferred.

Additional Opening for a second Geriatric Nurse Practitioner
Geriatric NP/Clinical Supervisor will often act on behalf of physician, performing diagnoses and treatment and interacting with Behavioral, Palliative Care and Transitional Care staff. You will also focus on documentation and information management related to the PACE Program. This will include case openings and 6-month reassessments. In addition, translation of documentation to a superbill with regard to coding guidelines linked to HCC coding.
