Job Title: Registered Nurse Field Supervisor
Department/Division: Administration
Reports to: Client Care Coordinator

Essential Job Responsibilities:
The RN Field Supervisor is responsible for the effective supervision of Care Partners.
Monitors the activity of Care Partners to ensure that services are being provided to clients in accordance with individualized service plans and state regulations.
v Responsible for the direct supervision of Care Partners
v Responsible for performing Quarterly Nursing Assessments for CBA clients.
v Responsible for in home orientation/training of Care Partners
v Performs all Supervisory visits in accordance with scheduled frequency
v Performs all Initial visits and post-hospital visits as assigned.
v Hire Care Partners in the field when required.
v Performs Complaint Investigations as assigned.
v Communicates with Client Coordinator daily
v Assists payroll with accurate documentation of missed time
v Reports to office twice weekly or more often as required.
v Completes CBA purchase request form and submits to Care Coordinator
v Responsible for signing all required CBA forms for purchases derived from RN assessment.
Knowledge, Skills and Abilities:
Strong organizational skills with excellent verbal communication skills. Must be able to work in a variety of environmental conditions. Must be sensitive to the needs of the aged and disabled and conscientious about servicing client’s needs. Knowledgeable of all State requirements under the Primary Home Care Program. Ability to handle crisis situations as well as do problem solving intervention. Ability to process time sensitive material in a timely manner.
