Description:
Spanish Speaking Nurse Practitioner wanted full-time for our wonderful modern primary care clinic.

In collaboration with physicians on the Medical Staff, provides care for adult and geriatric patients. This includes but is not limited to rounding on patients as directed by the MD or Hospital staff; writing routine orders in conjunction with the attending physician; reviewing test results and provides follow-up; completing paper work required for discharge.

Job Responsibilities:

§ Conducts patient rounds as necessary. Formulates and implements a therapeutic plan of care with physicians and staff. Writes in progress notes.
§ Assesses daily plan of care and recommends actions in collaboration with the patient’s physician.
§ Orders and interprets appropriate laboratory and diagnostic testing. Maintains contact with the ancillary testing areas for follow up.
§ Performs regular medication reviews for patients and modifies orders as necessary.
§ Recommends appropriate consults as needed.
§ Communicates with patient/family regarding plan of care and discharge plan.
§ Determines as necessary level of care i.e. ALC, denials.
§ Interacts with third party insurers regarding plan of care and continued stay.
§ Participates as needed in finalizing a safe discharge plan. Completes all required documentation.
§ Participates daily in rounds with the QRM staff. Assists staff in moving patient along clinical pathway.
§ Serves as a resource to the discharge planning staff.

Education, Training, Licensure and Experience Requirements

§ Board Certified AANP or ANCC
Masters Degree in Nursing from an accredited school.
§ Current NYS license to practice as a Registered Professional Nurse.
§ Current NYS certification as a Nurse Practitioner (either Adult/Geriatric).
§ Maintains a current collaborative practice agreement.
§ BCLS certification.
§ Experience in an acute care setting.
